NCLEX-PN
A wound measurement is useful only when another clinician can reproduce it and compare it with the patient's prior findings. The measurement therefore belongs in a larger clinical picture: identify why the tissue was injured, describe the wound and surrounding skin, record the intervention and patient response, and trend meaningful change over time. Size alone does not tell you whether a wound is healing or infected. A smaller surface area may coexist with increasing depth, new undermining, worsening pain, or ischemic edges. Conversely, a wound can look more exudative immediately after treatment changes without representing infection. The practical nurse's task is to obtain objective findings within state law, employer policy, training, and assigned responsibilities; recognize patterns that change priority; and communicate deterioration promptly.
