NCLEX-PN
An acutely red, hot, tender limb may represent a routine superficial infection, an abscess that needs source control, or a rapidly dangerous process beneath the skin. Erysipelas usually produces a bright, sharply outlined area because infection tracks through the upper dermis and superficial lymphatics. That visual pattern matters, but the first nursing decisions are broader: determine whether the infection is purulent, assess systemic stability, and look for signs that it has entered deeper tissue. For a US LVN/LPN, the practical work is to perform focused assessment, carry out prescribed treatment within state and facility requirements, document the infection’s trajectory, and report findings that change urgency. Antibiotics are not selected independently. The nurse’s clinical judgment is used to recognize the pattern, identify risks, and escalate when the pattern no longer fits uncomplicated erysipelas.
