Referral navigation is the clinical process of turning an identified need into a service the client can actually use. A resource list alone is not a completed referral. The nurse must help clarify the need, match the client with an appropriate program, transmit the required information, and confirm that the connection is feasible. The practical barriers are often more decisive than the diagnosis: transportation, cost, language, literacy, phone access, hours of operation, geographic distance, and caregiver availability can each prevent a correct referral from succeeding. A referral made at admission may no longer fit at discharge if the client's condition, functional status, or caregiver capacity changes. For Medicare-participating hospitals, discharge planning is a regulatory process rather than a courtesy. The hospital must identify patients likely to experience adverse health consequences without adequate planning, evaluate their discharge needs, and revise the plan when those needs change. The client or representative must be informed of freedom to choose among qualified participating post-discharge providers; the hospital may not simply select or restrict the available providers.
