NCLEX-PN
Common clinical procedures are safe only when the nurse links the task to its indication, patient identity, preparation, asepsis, and response. A catheter, IV, injection, suction pass, gastric tube, measurement, wound assessment, or restraint is not a routine task to complete on autopilot. The LPN/LVN uses the order and facility policy, confirms that the assignment is authorized by state law and within demonstrated competency, performs the technical steps, watches for harm, documents the result, and reports findings that require nursing judgment or a change in the plan. This lesson emphasizes the safety decisions that apply across procedures and the high-risk details that distinguish competent care from preventable injury.
