NCLEX-PN
An antibiotic order is not proof that an antibiotic is still needed. Stewardship is the coordinated process of using antibiotics only when indicated and, when they are indicated, selecting the correct agent, dose, route, and duration. The goal is not simply to give fewer doses. It is to achieve the best patient outcome while reducing adverse drug events, Clostridioides difficile infection, and selection of resistant organisms. At the bedside, stewardship is a repeated safety loop: clarify the infection being treated, collect information that can guide therapy, administer the ordered drug safely and on time, then reassess whether the plan still fits the patient's findings and results. A nurse does not independently diagnose the infection or choose a replacement antibiotic. The nurse does notice when the evidence no longer supports the current plan and communicates that change promptly.
