NCLEX-PN
An ethical concern becomes a patient-safety concern when a practice, policy, condition, or behavior could harm a patient or deny respectful, unbiased care. For the LPN/LVN, speaking up is not optional advocacy reserved for a senior nurse. It is a professional duty carried out within supervised practice: protect the patient from immediate harm, state what you observe and why it matters, notify the appropriate RN or supervisor, and continue escalation when the risk is not resolved. The safest response is neither silent compliance nor an unsupported accusation. It is a deliberate sequence: recognize the risk, intervene within immediate safety needs and authorized scope, communicate clearly, document objective facts, and use the next reporting level when necessary.
