The Pediatric Early Warning Score (PEWS) is a structured bedside tool for detecting clinical deterioration in hospitalised children. It combines three domains: behaviour or neurologic status, respiratory status, and cardiovascular or perfusion status. The score converts changes that a nurse can see, hear, and measure into a shared signal for timely escalation. PEWS is not a diagnosis. It does not tell you whether the child has bronchiolitis, sepsis, asthma, haemorrhage, medication-related respiratory depression, or another cause of illness. It tells you that the child’s current pattern may be becoming unsafe and that the response should become more urgent. Children compensate for physiologic stress differently than adults. Tachycardia and increased systemic vascular resistance can preserve blood pressure while tissue perfusion is already worsening. Once hypotension appears, compensation may be failing. That is why a child’s interaction, work of breathing, skin colour, and capillary refill deserve attention before a reassuring blood-pressure reading. Use the PEWS chart approved by the facility rather than reconstructing a score from memory. Item-level scoring and age-adjusted vital-sign ranges belong to the validated tool used in that setting....
