Community Resources and Referral Navigation

A community referral is not an administrative extra. It is a clinical response to a barrier that may otherwise make treatment impossible. A client may understand a medication plan yet be unable to follow it because food is unaffordable, transportation is unavailable, housing is unstable, or there is no caregiver to assist. A missed appointment may reflect the cost of travel rather than indifference. A wound-care plan may fail because the client has nowhere clean to store supplies. These social determinants of health affect whether the prescribed plan can be carried out and therefore affect health outcomes. The practical nurse assesses both the health problem and the conditions surrounding it. The useful question is not only, “Does the client understand the instructions?” It is also, “What could prevent this client from doing what we have asked?” That question reveals barriers that routine symptom assessment can miss.
