Community health nursing uses the nursing process with individuals, families, groups, and populations in settings such as homes, schools, shelters, primary-care clinics, workplaces, and public-health programs. The patient may be one person, but the cause of illness may involve a household exposure, unsafe housing, limited food access, or a gap in preventive care. The clinical question therefore extends beyond “What symptoms does this person have?” Ask three connected questions: - What immediate problem could harm this person now? - What exposure, behaviour, or social condition may be contributing? - What service, treatment, or follow-up would prevent the problem from recurring? The same fever can represent an uncomplicated viral illness in one person, an outbreak concern in a congregate setting, or sepsis in an older adult with new confusion and hypotension. Context changes the priority without replacing clinical assessment. An RPN—called an LPN in some provinces—collects assessment data, recognizes changes from baseline, implements authorized interventions, reinforces education, and communicates concerns.
