Atopic dermatitis is a chronic, relapsing inflammatory skin disease.
Atopic dermatitis is a chronic, relapsing inflammatory skin disease. Type-2 inflammation, driven largely by interleukins 4 and 13, weakens the epidermal barrier and produces intense pruritus. The damaged barrier loses water easily and allows irritants, allergens, and microbes to penetrate more readily. The result is a self-perpetuating itch-scratch cycle. Itching leads to scratching; scratching creates microtrauma, worsens inflammation, and further disrupts the barrier. Acute lesions may be erythematous, edematous, and weeping. Repeated scratching produces lichenification: thickened skin with exaggerated skin markings. Sleep loss, impaired concentration, excoriations, and widespread involvement often signal that the disease burden is substantial even when vital signs are normal. Emollients function like mortar in a damaged brick wall. They reduce water loss and help restore the barrier, but they do not adequately suppress an active inflammatory flare. Inflamed eczematous skin usually needs an anti-inflammatory medication in addition to regular moisturization.
