Overview
Why Phosphate Shifts Matter
Phosphate supports ATP production, cell membranes, bone mineralization, and oxygen delivery through red cell 2,3 DPG.
NCLEX-RN
NCLEX-RN
Phosphate supports ATP production, cell membranes, bone mineralization, and oxygen delivery through red-cell 2,3-DPG. The kidneys normally prevent excess phosphate from accumulating, so renal disease can push the serum level high. The opposite pattern appears when phosphate is continuously cleared by CRRT, shifts intracellularly during refeeding, or is wasted through a newly functioning kidney transplant. The number is only the beginning of the assessment. The urgency depends on how quickly the phosphate changed, whether calcium, potassium, or magnesium are also abnormal, whether kidney function is worsening, and whether the patient has respiratory, cardiac, or neurologic findings. A patient with chronic, stable hyperphosphatemia needs a different response from a patient with acute hyperphosphatemia, hypocalcemia, and AKI after chemotherapy.
