A Pediatric Early Warning Score (PEWS) is a bedside track-and-trigger system for hospitalized children. The nurse converts observed findings into points, adds the domain scores, and connects the total with a predefined response. It is not a diagnosis, a sepsis definition, or permission to wait when a child looks seriously ill. Most PEWS tools examine behavior or neurologic status, cardiovascular status, and respiratory status. These domains reflect the ways children commonly deteriorate: increasing work of breathing, impaired perfusion, and a change from normal interaction to irritability, lethargy, or reduced responsiveness. Because children can preserve blood pressure until shock is advanced, a rising heart rate, prolonged capillary refill, color change, or altered behavior may require action before hypotension appears. Use the validated tool adopted by the facility, with age-appropriate vital-sign ranges. A rising score, a concerning single finding, staff concern, or caregiver concern should prompt reassessment and escalation even when the calculated total is low.
