Postpartum hemorrhage remains a leading cause of maternal morbidity and mortality, and when uterine atony causes excessive bleeding, uterotonic medications contract the uterus to reduce blood loss. After oxytocin, the first-line agent, the nurse must know two additional uterotonics, methylergonovine and carboprost, because each has a specific contraindication that determines which client can safely receive it. Selecting and monitoring these drugs correctly is a high-stakes nursing responsibility. The two second-line agents are mirror images in their cautions. Methylergonovine, an ergot alkaloid, causes vasoconstriction and raises blood pressure, so it is withheld in hypertension. Carboprost, a prostaglandin, causes bronchoconstriction, so it is withheld in asthma. Knowing which client cannot receive which drug is the core skill. Consider a case: you are the nurse caring for a client with postpartum hemorrhage from uterine atony whose blood pressure is 168 over 104, and the provider considers methylergonovine. Which action is the priority? You recognize that methylergonovine is contraindicated in hypertension, report the blood pressure, and hold the drug, because its vasoconstriction would dangerously worsen her pressure and could precipitate stroke or myocardial ischemia...
