NCLEX-RN
A fall usually occurs when a patient’s immediate need exceeds the patient’s ability to meet it safely. Pain, urgency to void, dizziness, delirium, weakness, unfamiliar surroundings, and attached equipment can turn an ordinary transfer into a hazardous one. This topic rewards connecting assessment data to risk, choosing the first action that prevents additional harm, and recognizing which tasks may be delegated while the RN retains clinical judgment. Purposeful hourly rounding reduces the need for patients to get up alone, but it does not replace individualized observation, mobility assistance, or reassessment after a change in condition.
