Congenital Hypothyroidism

Congenital hypothyroidism is inadequate thyroid hormone production present at birth. It most often results from thyroid dysgenesis, an ectopic thyroid gland, or a defect in thyroid hormone synthesis (dyshormonogenesis). Thyroid hormone drives early brain maturation, linear growth, and skeletal development. In the first months of life, the developing brain needs a reliable hormone supply; untreated deficiency can cause permanent neurodevelopmental impairment. The treatment goal is therefore not merely to normalize a laboratory value. It is to restore thyroid hormone early enough to protect development. The clinical trap is that many affected newborns initially look well. Maternal thyroid hormone crossing the placenta can partially mask the infant’s deficiency for a short time. Newborn screening finds the condition before the outward picture becomes obvious.
