What this practice tool does

Dosage calculation practice generates realistic nursing-school scenarios for tablets and capsules, oral liquids, injections, weight-based dosing, IV pump rates, IV drip rates, reconstitution, insulin, heparin, and unit conversions. After you enter an answer, it shows a step-by-step solution, the formula used, the rounding rule, a common mistake, and a clinical safety note.

This is a study tool, not a clinical dosing tool. It does not give real patient-specific dosing advice. Always verify real medications with the order, MAR, institutional policy, and pharmacist/provider guidance.

Core dosage formulas

Amount to give

Amount = (dose ordered ÷ dose available) × quantity — the same setup whether the quantity is one tablet or the millilitres in one dose of a liquid or injection.

IV flow rates

mL/hr = total volume ÷ time (hours), and gtt/min = (volume × drop factor) ÷ time in minutes. Convert hours to minutes before finding drops per minute.

Weight-based dosing

mg/dose = weight (kg) × mg/kg/dose, and for a divided daily dose, mg/dose = (weight × mg/kg/day) ÷ number of doses. Convert pounds to kilograms first (kg = lb ÷ 2.2).

Step-by-step example

Order: Infuse 1,000 mL over 8 hours by pump. Find: the rate in mL/hr.

  1. Identify volume = 1,000 mL and time = 8 hr.
  2. Apply mL/hr = volume ÷ time = 1,000 ÷ 8.
  3. Result = 125 mL/hr; round to whole mL/hr for most pumps.

Common mistakes & exam traps

  • Time units: using hours instead of minutes when finding gtt/min.
  • Inverted ratios: dividing available by ordered instead of ordered by available.
  • Rounding drift: rounding too early and carrying the error into later steps.
  • Unit confusion: mixing mg and mcg, or treating insulin units as millilitres.
