NCLEX-RN
Most benign nevi remain relatively stable. A lesion that changes, differs markedly from the patient's other lesions, or develops persistent bleeding or ulceration shifts the nursing task from simple description to risk recognition and referral. ABCDE provides a consistent way to recognize suspicious pigmented patterns, but appearance alone cannot establish histology. The RN assesses the lesion and the patient's risk, documents the findings precisely, and ensures timely evaluation through the authorized-provider or dermatology pathway.
