NCLEX-RN
Calcium and phosphate abnormalities can change nerve, muscle, cardiac, and skeletal function. The immediate nursing question is not simply whether a value is high or low; it is whether the imbalance is producing neuromuscular irritability, dysrhythmia, airway compromise, altered mental status, or impaired perfusion. The calcium–phosphate relationship is especially clinically relevant in chronic kidney disease (CKD). Reduced phosphate excretion creates persistent phosphate retention, while altered vitamin D activation and parathyroid hormone regulation disturb calcium balance. Interpret the laboratory result alongside the albumin level, ionized calcium when available, symptoms, ECG, kidney function, and the direction of the trend. A symptomatic patient or a severe value requires action before routine education or medication titration.
