NCLEX-RN
Bowel and bladder care is often treated as routine comfort work, yet changes in elimination may be the earliest sign of infection, obstruction, bleeding, medication toxicity, neurologic compromise, dehydration, or acute kidney injury. Safe nursing care distinguishes expected variation from a pattern that threatens perfusion, continence, skin integrity, or organ function. The goal is not to make every patient void or defecate on a schedule. It is to understand the patient's baseline, protect dignity, remove reversible barriers, and recognize when a symptom needs focused assessment rather than another pad, laxative, or catheter.
