End-of-life care is the one episode of nursing that cannot be repeated or corrected afterwards. How a person's final days are managed determines whether they die with symptoms controlled and dignity intact, and it shapes the bereavement of everyone who watched. The clinical stakes are concrete: unrelieved pain, breathlessness, agitation, and noisy secretions are common, are frightening to witness, and are almost always treatable. Yet they are routinely undertreated, largely because clinicians fear that adequate symptom relief will hasten death — a fear that decades of evidence do not support when medication is titrated to symptom. The other recurring failure is the opposite one: continuing burdensome investigations and interventions that cannot change the outcome, because nobody has had the conversation about what the patient actually wants. Comfort care is not the absence of treatment. It is a deliberate, active plan whose goal has shifted from prolonging life to relieving suffering, and it demands the same rigour of assessment, titration, and reassessment as any critical care.
