REx-PN
Therapeutic communication is a clinical intervention, not simply a pleasant conversation. The nurse’s words, pace, silence, attention, and boundaries determine whether a client feels safe enough to describe thoughts, emotions, symptoms, and risk accurately. In mental-health care, communication performs two jobs at once: it builds a collaborative relationship and provides much of the information used for the Mental Status Examination. A client who appears “uncooperative” may instead be frightened, sedated, unable to hear, struggling with language, or overwhelmed by the environment. The experienced nurse checks for these barriers before interpreting the response as a behavioural problem. Use therapeutic communication before routine pharmacologic intervention to establish engagement and gather mental-status and safety information. Do not delay emergency treatment when a client presents an immediate threat to self or others; communicate calmly while activating the required safety response.
