Rotavirus is a highly contagious, fecal–oral viral infection that primarily causes gastroenteritis in infants and young children.
Rotavirus is a highly contagious, fecal–oral viral infection that primarily causes gastroenteritis in infants and young children. After an incubation period of approximately 18–72 hours, the child commonly develops fever and vomiting followed by frequent, watery, usually non-bloody diarrhea. Symptoms generally last 3–7 days. The virus is non-enveloped and contains 11 segments of double-stranded RNA. Its outer capsid proteins, VP4 and VP7, help determine serotype and are targets in vaccine development. Clinically, the more useful mechanism is what the virus does to the small intestine: it injures mature absorptive enterocytes on the villi. The damaged surface absorbs less sodium, water, and nutrients, so fluid remains in the intestinal lumen and is lost in stool. Repeated vomiting adds another route of fluid loss. A young child has little reserve. A relatively short period of vomiting and diarrhea can therefore produce a meaningful fall in circulating volume and electrolyte imbalance. The immediate nursing threat is not identifying the viral strain; it is recognizing whether the child is still perfusing and replacing the losses safely.
