A ventilator bundle prevents ventilator-associated pneumonia (VAP) by addressing the hazards created by an artificial airway, not by “cleaning the ventilator.” VAP usually develops after at least 48 hours of invasive ventilation when bacteria reach the lower respiratory tract and establish infection. The endotracheal tube bypasses the upper airway’s filtering function, weakens cough, and impairs mucociliary clearance. It also becomes a surface for bacterial biofilm. Most importantly, oropharyngeal and gastric secretions collect above the cuff. Even an inflated cuff does not create a perfect seal; small volumes can leak around it and be aspirated into the lungs. The prevention logic follows this pathway: - Reduce aspiration of secretions and gastric contents. - Remove pooled secretions where possible. - Limit bacterial burden in the mouth without causing harm. - Avoid intubation when a safe noninvasive option is suitable. - Shorten the time the tube remains in place. The most effective preventive intervention is to avoid intubation when safely possible, such as by using high-flow nasal cannula oxygen or noninvasive positive-pressure ventilation in an appropriate patient.
