Priapism

Priapism is a penile erection lasting longer than 4 hours without sexual stimulation. The urgent clinical decision is whether blood is trapped in the corpora cavernosa (ischemic, low-flow) or whether trauma has created excess arterial inflow (nonischemic, high-flow). The first pattern threatens smooth muscle and erectile function; the second is usually less urgent. Pain, rigidity, duration, and a soft glans help establish priority while corporal blood gas confirms the type.
