Pediatric Well-Visit Red Flags

A preventive visit is designed to find problems before they become crises, but “well” describes the appointment, not a guarantee about the child. A child may be alert and feeding while harbouring invasive infection, a bleeding disorder, developmental regression, or an injury that requires immediate protection. For children from birth to age 5, the Rourke Baby Record provides the Canadian structure for the visit: growth and nutrition, developmental surveillance, physical examination, immunization review, and anticipatory guidance. The 2024 revision also emphasises early relational health, inclusive and culturally safe care, injury prevention, healthy routines, and clearer developmental surveillance. These elements are connected. A change in feeding may explain faltering growth; a caregiver’s concern may be the first sign of hearing loss; an unexplained bruise may change the entire safety assessment. An experienced NP notices the child’s trajectory before interpreting an isolated measurement. The first questions are often simple: What has changed? What does the caregiver think is wrong? Can the child do what they previously could do? Is the explanation for an injury compatible with the child’s mobility? A red flag...
