A comprehensive H&P starts with a safety decision, not with a form. Before pursuing a detailed symptom history, look at appearance, work of breathing, circulation, mental status, and vital-sign trajectory. A client who is newly unstable needs immediate help and emergency management according to local policy; pause routine data collection, support airway, breathing, and circulation within scope, reassess the response, and communicate the change. One assessment is a snapshot, so a worsening trajectory can change the priority even when an earlier examination was reassuring. Canadian nursing assessment practice is both holistic and bounded by authority. Comprehensive assessment across the lifespan is an entry-level competency for RNs and RPNs, and the data set should account for physical, psychological, social, cultural, and spiritual dimensions.
